(Picking up from last season's finale) Joe Carroll's acolyte Molly creeps in to Ryan's apartment and surprises him, stabbing him twice in the stomach. Claire comes out of the bathroom and before Ryan can warn her, Molly stabs her through the back. Ryan manages to pull Molly off Claire, but she's already badly wounded.
Later, paramedics bring Ryan to the hospital where his only concern is Claire. When he wakes up, a stricken Mike comes to Ryan's room to give him the bad news -- Claire is dead.
One Year Later Ryan wakes up and goes for a jog through Central Park. When he comes back to his light and airy new apartment, he sees a news broadcast about a book called "The Havenport Tragedy" about last year's murders, by Carrie Cook.
Ryan goes to an AA meeting. He's five months sober. His sponsor (Keith Carradine) urges him to share next time.
New York College of Criminal Justice Ryan teaches a class, asking his students to analyze a murder scene.
Later that night, Ryan's niece, Max, brings her pastry chef boyfriend, Chris, to dinner. His niece is in the Intelligence Unit of the NYPD. Ryan's sponsor, Barry, and Lisa from the AA meeting, join them.
In the kitchen, Max confesses she's trying to keep Ryan distracted during the anniversary week of the cult murders.
Union Square Station On a subway train, a blonde woman texts as a man in a Joe Carroll mask walks onto the car. Then two more come on. One peers into the surveillance camera and they all just stand there. People on the car get nervous and then the Joes start shouting "Ressurrection!" and attacking riders. They get several, who try to help each other, but can't escape the knife-wielding attackers. They shout: "Joe Carroll lives! Ryan Hardy can't stop us!" The blonde texter lives because the train reaches the next stop and the Joes get off.
In the morning, Ryan sees the news. Five dead. He hears the reports asking if they think it's the work of the Carroll Cult.
Woodbridge, Virgina Mike is sitting at home in his boxers when he sees FBI Agents pull up. He's was suspended pending review, but they want him in New York to consult.
An attractive young blonde woman returns home to her apartment and sees a young man in her lobby. She smiles nicely at him and thinks he's cute, and gets on the elevator. She finds him significantly less cute when she sees him standing in front of her door.
Ryan arrives at his class and tells the gathered throng of reporters that he's not involved in the case. Then he gets to his classroom and sees police waiting for him.
At the scene, Special Agent Gina Mendez introduces herself to Mike and Agent Phillips. The three perps used a fire exit to escape.
Back at the stalker apartment, the young man lies in bed with the young woman and talks to her, saying he doesn't want to have sex. Then we see that the woman he's talking to is dead, with strangulation marks around her neck. He keeps talking to Heather as he goes to the kitchen to make food.
At the hospital, Ryan and Mike see each other for the first time in eight months. Flash back to the last time was at a bar, when Ryan was hammered. Mike says Joey is in witness protection with his grandmother. But Mike is worried about his testimony in front of a grand jury.
He tells Ryan what to say about the shooting in the woods, when Ryan shot an unarmed Carroll acolyte out of anger. Mike will say the man's cuffs were off and he lunged at Ryan, all Ryan has to say is he doesn't remember.
Ryan doesn't seem inclined to take the lifeline Mike is offering him.
Mike asks him if he remembers Debra's last words, clearly disappointed in Ryan.
Back in the present, they say a cordial hello. Mendez asks Ryan if he knows anything that can help them, he says no.
The surviving victim is Lily Gray, an art dealer. Phillips, Mendez, Mike and Ryan go talk to her. She tells them what the killers said.
Back in Heather's apartment, her killer makes himself at home, listening to her music and going through her things. Then he dances with her corpse and later does her hair and make-up. He showers, then prepares to get Heather ready for something. But first he goes back to the bathroom where his accomplice is finishing up a shower -- his identical twin.
Burlington County, New Jersey A totally punked out Emma sees the report about the subway attack and is angry. Her friend tells her that Frank thinks Carlos might be responsible.
At the FBI building in NYC, Mendez explains that only 40% of subways have cameras and they're only in every other car, so the killers wanted to be filmed. The masks are hand painted molded silicone, very high end.
Mike pulls up info on the eight members of the Havenport cult they're keeping tabs on. One of them matches one of the killers physically: Carlos Perez.
The FBI tires to figure out if they're dealing with a new cult, they turn to Ryan for help. He "aw shucks" his way out of the meeting and says he hasn't kept up with the case. He leaves. Mike follows, incredulous that Ryan is leaving. But Ryan says he's done.
Ryan goes home, to a keypad protected room in his apartment, and we see that he was lying. The room is chockfull of his investigation into Carroll and his cult, and a small arsenal. He pulls out his file on Carlos.
Later, Ryan's niece Max comes by, ready to work. She's been helping him, and tracked Carlos to New York. The FBI brought her Intelligence Unit into the case, but she didn't volunteer, remembering she's part of the Ryan Hardy Secret Task Force. She logs onto her NYPD system and, using a combination of Carlos' old aliases, finds his signature on a short term lease in the city. Max urges Ryan to go to the FBI, but he just asks her to lock up when she leaves.
Ryan goes to Carlos' apartment and picks the lock. He jumps on Carlos when he comes home and accidentally knocks him out. He wakes him up and asks here Joe is. Carlos insists he's dead, but Ryan doesn't believe it for a second. With Ryan straddling him on the floor, Carlos starts talking as he reaches behind Ryan under the couch.
He says he hasn't see Joe since he picked him up from the beach after the boat house explosion. He drove him as far as a road block, but hasn't seen him since.
Ryan stops him from getting the gun under the couch, but as he's doing so, he's distracted and Carlos grabs something with his other hand and clocks Ryan. Carlos leaps up and races out of his apartment, down the stairs and out the door.
Ryan chases him down the dark city street but loses him when a cab nails Ryan as he's racing across the street.
Ryan staggers to his feet and gets up.
Carlos goes to a midtown apartment where a French woman is putting numbers on burner cell phones.
Ryan goes back to Carlos' apartment and looks through his things. He finds the Joe Carroll mask. He also finds a playbill with a picture of Heather Clarke circled. Ryan calls Max for info on Heather and learns that she was found dead 45 minutes ago.
As Ryan leaves Carlos' apartment, Emma comes out from the shadows and goes upstairs.
Heather's body was left sitting perfectly upright, tied to a park bench, reading Joe's book, dressed like the character based on Claire.
Mike asks Ryan for his insight, but Ryan insists he has none. Mike asks why Ryan requested the full DNA report on Joe -- his DNA matched Joey's. Mike insists Joe is dead and Ryan agrees. Mike asks why Ryan didn't return his calls, and bares his soul over wishing things had gone differently with Debra, that they had found her in time. But Ryan just tells him to take care of himself, and leaves.
In the hotel room, Carlos sees his mug shot all over the TV. The French woman answers the knock on the door -- it's the twins, Luke and Mark. The French woman thinks Carlos is dead.
Luke blames Carlos for getting caught when he said his apartment was untraceable. Mark tells his brother to let it go. Luke goes from angry to laughing in an instant and says it's fine. He wants to hug Carlos, in the creepiest way possible. Carlos is good without the hug. Luke chops Carlos in the windpipe as the French woman laughs and Mark looks on disapprovingly. Carlos flops on the bed, gasping for air, but will live.
Ryan goes to the hospital to visit Lily, the subway survivor, to apologize.
Emma returns to her house and tells her friend Hannah that she didn't find Carlos, but she did find Ryan, and she has no idea what's going on.
Back with the creep twins, Luke is making Carlos stand in time-out in the corner, while Mark reads a book about the Havenport tragedy.
Back in his secret Joe Carroll war room, Ryan looks at a picture of Claire.
At a house in the woods, a man pays a woman named Judy for sex. As he walks out her living room, a young teenage black girl does homework on the couch. She calls for a man named Daryl when she sees the news reports on the subway killings. Daryl, in a lumberjack coat and trucker hat, comes into the house to see Ryan on the TV. It's Joe Carroll, with a thick beard.
At his house, Ryan tries on the Joe Carroll mask, looks in the mirror, and doesn't like what he sees.
